Future Leaders Communiqué October 2020

The October 2020 edition of Future Leaders Communiqué covers clinical documentation and the important role it plays in patient care. This edition presents case studies and the risks behind insufficient patient documentation and informal consultations, such as decreased quality of care. The final commentary also reminds junior doctors of the legal implications behind appropriate documentation. The commentaries also offer recommendations for clinicians to improve recording writing, through exercising caution, practice and the SOAP (subjective, objective, assessment, plan) system. As clinicians continue to critically evaluate their own and each other’s records and handover notes, this will lead to further learning and development and enhanced patient safety.
